Epidural Blockade for Cancer Pain


Abstract
For the patient with cancer, pain is one of the most feared consequences. For the clinician, pain is a difficult diagnostic and therapeutic medical problem. Advances in the diagnosis and treatment of cancer, coupled with recent advances in our understanding of the anatomy, physiology, pharmacology, and psychology of pain perception, have led to improved care of the patient with pain of malignant origin. Specialized methods of cancer diagnosis and treatment provide the most direct approach to treating cancer pain by treating the cause of pain. However, if treatment of the cause of the pain has failed or injury to the bone, soft tissue, or nerve has occurred as a result of therapy, appropriate management of pain is essential.
The goal of pain therapy for patients receiving active treatment is to provide them with sufficient relief to tolerate the diagnostic and therapeutic approaches required to treat their cancer.
Epidural nerve block with local anesthetics, neurolytics, and opioids is safe and effective. For patients with advanced disease, epidural nerve block should allow them to function at level they choose and to die relatively free of pain.
